How to Report
If you believe someone is breaking our Code of Conduct, please report any violations ASAP. Depending on the incident, email: info@coflowco.com.
If you need a 3rd party to report to, we understand. Maybe you feel unsafe addressing it within our organization or it would be a conflict of our interest and we couldn’t remain impartial. If so, you may reach out to Shoshana Rosenberg, J.D. at Safe Porter. She will provide other contacts to ensure your issue is handled properly and taken seriously. All reports will be kept confidential regardless of reporting mechanism.
Please see the Code section for incidents determined to violate our Code of Conduct.
If you’re unsure if an incident is in violation of the Code of Conduct or not, please report it anyway. This helps us not miss any actual violations and helps us update our Code of Conduct, as we grow our understanding of types of uncertain communications that may occur.
Please submit the following via email or record documentation:
Your email and phone number:
Name of individual(s) involved in the incident (real names, nicknames, and pseudonyms):
Witness(es) of incident:
Date, time, location of incident:
Share your account of the incident, in as much detail as you can muster.
Please include any context that may have also occurred outside of the stated incident, and any other information you think we should have.
Is this incident ongoing?
Please include any screenshots or links to online incidents.
After submittal, the 3rd party will acknowledge receipt within 24 hours or sooner.
The coFLOWco Ethics + Justice Working Group (to be formed) will meet to review the incident and determine if the incident violates our Code of Conduct if it falls within their duties. We may contact law enforcement if we believe there is a physical threat of danger, and your safety comes first.
We encourage anyone submitting a report to contact law enforcement immediately if you believe there is a physical threat of danger to you or anyone else.
If your report concerns someone who is part of the Leadership team or the Ethics + Justice Working Group, please see alternative contacts in our Employee Guidebook.
You may also leave anonymous complaints, concerns, or feedback about the Collective via this form. (No sign-in needed).
